Integrated Health 21 is pleased to be working with Mary Mother of God  to facilitate COVID-19 vaccine administration. We will be administering the 2nd Moderna COVID-19 Vaccine to Mary, Mother of God parishioners, family members and all our neighbors who qualify, on Date to be determined.
 
We will be offering to the 65+ group and the availability of additional vaccine we are now able to offer available spots at this clinic to Persons age 18-64 with high-risk conditions, as defined by the Pennsylvania Department of Health. In order to meet this eligibility criteria, please review the following conditions outlined by the Pennsylvania Department of Health: 
 
Person's age 18-64 with high-risk conditions:
Cancer , Chronic kidney disease, COPD, Down Syndrome, Heart conditions, such as heart failure, coronary artery disease, or cardiomyopathies , Immunocompromised state (weakened immune system) from solid organ transplant or from blood or bone marrow transplant, immune deficiencies, HIV, use of corticosteroids, or use of other immune weakening medicines , Obesity (body mass index [BMI] of 30 kg/m2 or higher but < 40 kg/m2) , Severe Obesity (BMI ≥ 40 kg/m2) , Pregnancy, Sickle cell disease, Smoking, Type 2 diabetes mellitus

Vaccine supplies are limited, so scheduling is on a first come first serve basis. If you are not interested in receiving the vaccine or have already received your vaccine, please notify Integrated Health 21 by replying to this email.

Please Note:

• When scheduling, if you have received any other type of vaccine (shingles, pneumonia, etc.) you must wait at least two weeks between that vaccine and receiving the COVID-19 vaccine.

• Details regarding scheduling your second shot will be provided when you are vaccinated.

• Individuals with concerns about their health and whether they should receive the vaccine should contact their personal Primary Health Care Provider.

• Attached is a fact sheet regarding the Moderna Vaccine for your review.

• STAY SAFE: If you have any of the following symptoms, please reschedule your appointment:  

Fever , Cough, Shortness of breath, Sore throat, Chills or Body Aches, Diarrhea, Nausea or vomiting.
